How can I get a copy of my medical records?
Download the Authorization to Release and Disclose Health Information form and return it to us by email, fax or mail. You may request your records in paper or electronic format.

Download the Authorization to Release and Disclose Patient Information form and return it to us. Forms may be received via email, fax, or mail (see information above for email, fax, or address).
The time will vary depending on who is requesting your records. The turnaround time is as follows:
Federal and State law allow healthcare providers to process requests within 30 days of receiving the request. Most patient requests are processed within 2 business days of receiving a completed Authorization to Release and Disclose Health Information form.
You will need to get the authorization of the patient and/or guardian to request records. Print Authorization to Release and Disclose Patient Information form.
